FIRST-LINE AFATINIB COMBINED WITH GAMMA KNIFE FOR BRAIN METASTASES IN EGFR-MUTANT NON-SMALL CELL LUNG CANCER PATIENTS: TREATMENT OUTCOME

Mục tiêu: Đánh giá tỷ lệ đáp ứng (tổng thể và nội sọ), sống thêm không tiến triển nội sọ và sống thêm không bệnh tiến triển chung.

Thiết kế: Nghiên cứu mô tả hồi cứu trên bệnh nhân ung thư phổi không tế bào nhỏ (UTPKTBN) di căn não, có đột biến EGFR, tại Bệnh viện K từ 1/2018 đến 10/2024, điều trị afatinib kết hợp gamma knife.

Kết quả: 44 bệnh nhân được tuyển chọn. Tỷ lệ đáp ứng tổng thể đạt 72,7% trong đó không có đáp ứng hoàn toàn, 72,7% đáp ứng 1 phần. Tỷ lệ đáp ứng tại não đạt 72,7% trong đó đáp ứng hoàn toàn đạt 22,7% và đáp ứng 1 phần là 50%. Trung vị thời gian sống thêm không bệnh tiến triển (PFS) đạt 12,9 tháng (10,2-19,2). Trung vị thời gian sống thêm không tiến triển nội sọ (iPFS) đạt 28,2 tháng (14,7-NR). Bệnh nhân mang đột biến thường gặp (Del19/L858R) có iPFS trung vị cao hơn đáng kể so với nhóm mang đột biến hiếm (34,4 tháng so với 17,0 tháng; p = 0,019).

Kết luận: Kết hợp gamma knife và TKIs thế hệ 2 (afatinib) cho hiệu quả điều trị tốt về đáp ứng, PFS, iPFS trên bệnh nhân UTPKTBN, di căn não có đột biến EGFR.

Abstract

Objective: To evaluate the response rate (overall and intracranial), intracranial progression-free survival (iPFS), and overall progression-free survival (PFS).

Methods: This retrospective descriptive study included patients diagnosed with EGFR-mutant non-small cell lung cancer (NSCLC) with brain metastases, who received treatment with afatinib in combination with gamma knife radiosurgery at the National Cancer Hospital between January 2018 and October 2024.

Results: A total of 44 patients were included. The overall response rate (ORR) was 72.7%, with no complete responses and 72.7% partial responses. The intracranial response rate (iORR) was also 72.7%, including 22.7% complete responses and 50% partial responses. The median PFS was 12.9 months (95% CI, 10.2–19.2), and the median iPFS was 28.2 months (95% CI, 14.7–not reached). Patients with common EGFR mutations (Del19/L858R) had a significantly longer median iPFS compared to those with uncommon mutations (34.4 vs. 17.0 months; p = 0.019).

Conclusion: The combination of gamma knife radiosurgery and second-generation TKIs (afatinib) demonstrated favorable efficacy in terms of response rate, PFS, and iPFS in patients with EGFR-mutant NSCLC and brain metastases.
