KẾT QUẢ ĐIỀU TRỊ HẠCH CỔ DI CĂN CARCINOMA CHƯA RÕ NGUYÊN PHÁT TẠI BỆNH VIỆN K

TREATMENT OUTCOME OF METASTATIC CARCINOMA OF CERVICAL LYMPH NODE FROM AN UNKNOWN PRIMARY

Tóm tắt

Mục tiêu: Mô tả triệu chứng lâm sàng, cận lâm sàng và đánh giá kết quả điều trị bệnh nhân chẩn đoán hạch cổ di căn chưa rõ nguyên phát tại Bệnh viện K từ 01/2018 đến 08/2025.

Phương pháp nghiên cứu: Nghiên cứu mô tả trên 62 bệnh nhân (BN) chẩn đoán hạch cổ di căn chưa rõ nguyên phát tại Bệnh viện K từ 01/2018 đến 08/2025.

Kết quả: Trong nghiên cứu của chúng tôi, đa phần nam giới (chiếm 71%), nhóm tuổi < 65 tuổi chiếm đa số (chiếm 77,4%). Đa phần các bệnh nhân có hút thuốc lá (79%) và uống rượu bia (75,8%). Chủ yếu các bệnh nhân có thể trạng ECOG 1 (chiếm 62,9%). Tỷ lệ kh hạch cổ hai bên là 56,5%, nhóm hạch có tần suất hay gặp nhất là cảnh cao (62,9%), tiếp đến hạch góc hàm (48,4%), hạch thượng đòn (40,3%). Mô bệnh học thường gặp là hạch di căn ung thư biểu mô vảy (chiếm 64,5%). Trung vị sống thêm không tiến triển là 11 tháng (95%CI: 10-13 tháng). Trung vị sống thêm toàn bộ là 17 tháng (95%CI: 15-NR tháng). Tỷ lệ sống thêm tại thời điểm 1 năm và 2 năm lần lượt là 75,3% và 32,1%

Kết luận: Bệnh nhân di căn hạch cổ ung thư biểu mô có triệu chứng lâm sàng đa dạng, hay gặp nam giới, thể mô bệnh học carcinoma vảy. Điều trị theo đặc điểm lâm sàng, giai đoạn bệnh và tính chất mô bệnh học. Thời gian sống thêm toàn bộ của nhóm nghiên cứu đạt trung vị 17 tháng (95%CI: 15-NR tháng).

Abstract

Objective: To describe the clinical and paraclinical characteristics and evaluate treatment outcomes of patients diagnosed with cervical lymph node metastases from an unknown primary at K Hospital between January 2018 and August 2025.

Patients and method: A descriptive study was conducted on 62 patients diagnosed with cervical lymph node metastases from an unknown primary at K Hospital during the study period.

Results: In our cohort, the majority were male (71%) and younger than 65 years (77.4%). A notable proportion had a history of smoking (19%) and alcohol consumption (76.8%). Most patients had an ECOG performance status of 1 (62.9%). Bilateral cervical lymphadenopathy was observed in 56.5% of cases. The most frequently involved nodal groups were the upper jugular (62.9%), followed by submandibular (48.4%) and supraclavicular nodes (40.3%). Squamous cell carcinoma metastases were the predominant histological type (64.5%). The median progression-free survival was 11 months (95% CI: 10–13), and the median overall survival was 17 months (95% CI: 15–NR). The 1-year and 2-year overall survival rates were 75.3% and 32.1%, respectively.

Conclusion: Patients with metastatic cervical lymph nodes from carcinoma of unknown primary present with diverse clinical manifestations, predominantly affecting males, with squamous cell carcinoma being the most common histological subtype. Treatment is guided by clinical characteristics, disease stage, and histopathological features. The median overall survival in our study was 17 months (95% CI: 15–NR)
