EVALUATION OF RECONSTRUCTION RESULTS FOR TOTAL GLOSSECTOMY DEFECTS USING RECTUS ABDOMINIS FREE FLAP IN THE TREATMENT OF TONGUE AND FLOOR OF MOUTH CANCER
Điều trị bệnh nhân ung thư lưỡi giai đoạn trễ với cắt toàn bộ lưỡi tạo ra thách thức cho phẫu thuật tái tạo. Vạt da cơ thẳng bụng là một trong những lựa chọn được ưa thích hiện nay đặc biệt đối với bệnh nhân có thể trạng gầy. Có 30 bệnh nhân chẩn đoán ung thư lưỡi tiến xa được cắt toàn bộ lưỡi và tái tạo bằng vạt da cơ thẳng bụng. Có 1 trường hợp hoại tử một phần vạt với tỷ lệ chuyển vạt thành công là 100%. Tỷ lệ sống còn không bệnh sau 2 năm là 60%. Sự phục hồi chức năng nói và nuốt là chấp nhận được. Kết quả cho thấy vạt da cơ thẳng bụng là lựa chọn tốt cho tạo hình cắt toàn bộ lưỡi đặc biệt là những bệnh nhân có thể trạng gầy.
Abstract
Treating advanced-stage tongue cancer patients with total glossectomy presents significant challenges for reconstructive surgery. The rectus abdominis free flap is currently one of the preferred options, especially for patients with a thin body build. 30 patients diagnosed with advanced tongue cancer underwent total tongue resection and reconstruction using this flap. There was one case of partial flap necrosis, with a 100% flap transfer success rate. The two-year disease-free survival rate was 60%. Functional recovery of speech and swallowing was acceptable. We believe that the rectus abdominis free flap is a good option for total tongue reconstruction, particularly in patients with a slim physique.
