RENAL MALAKOPLAKIA MASQUERADING AS MALIGNANCY ON IMAGING: A RARE CASE REPORT AND LITERATURE REVIEW

Malakoplakia là một bệnh viêm u hạt hiếm gặp, đặc trưng bởi sự hiện diện của các đại thực bào chứa thể vùi Michaelis-Gutmann. Tổn thương tại thận đặc biệt hiếm và dễ bị nhầm lẫn với khối u ác tính trên hình ảnh học. Chúng tôi báo cáo trường hợp bệnh nhân nữ, 62 tuổi, tiền sử đái tháo đường type 2, nhập viện vì đau âm ỉ vùng hông lưng trái kéo dài một tháng. Hình ảnh CT bụng ghi nhận khối nghi ngờ ác tính ở thận trái, do đó chỉ định cắt thận được đưa ra. Tuy nhiên, mô bệnh học sau mổ cho thấy sự tăng sinh lan tỏa của mô bào, thấm nhập tế bào viêm hỗn hợp và sự hiện diện của các thể vùi Michaelis-Gutmann đặc trưng. Mô bệnh học với nhuộm PAS, Von Kossa, CD68 và CD163 đã hỗ trợ xác định chẩn đoán. Bệnh nhân được điều trị tiếp theo với kháng sinh ciprofloxacin trong vòng 1 tháng và khỏi bệnh hoàn toàn sau 6 tháng. Từ đó, chúng tôi nhận thấy vai trò thiết yếu của khảo sát mô học trong việc chẩn đoán xác định malakoplakia, đặc biệt khi tổn thương xảy ra ở vị trí hiếm gặp và dễ gây nhầm lẫn với ung thư.

Abstract

Malakoplakia is a rare granulomatous inflammatory disease characterized by the presence of macrophages containing Michaelis-Gutmann bodies. Renal involvement is particularly uncommon and often mimics malignancy on imaging studies. We report the case of a 62-year-old woman with a history of type 2 diabetes mellitus who presented with dull left flank pain persisting for one month. Abdominal CT scan revealed a suspicious mass in the left kidney, prompting a nephrectomy under the initial impression of renal malignancy. However, histopathological examination showed diffuse proliferation of histiocytes, mixed inflammatory cell infiltration, and characteristic Michaelis-Gutmann bodies. Special stains including PAS and Von Kossa, along with immunohistochemical markers CD68 and CD163, confirmed the diagnosis of malakoplakia. The patient received ciprofloxacin for one month, with complete resolution observed after six months. This case underscores the crucial role of histopathology in diagnosing malakoplakia, especially when occurring at rare anatomical sites that may mimic cancer radiologically and clinically.
