CONCURRENT CHEMORADIATION THERAPY FOR PRIMARY SQUAMOUS CELL CARCINOMA OF THE URETHRA: A RARE CASE REPORT AND LITERATURE REVIEW

Giới thiệu: Ung thư niệu đạo nguyên phát là bệnh lý hiếm gặp, chiếm dưới 1% ung thư hệ niệu dục. Bệnh thường dễ cho di căn hạch tại thời điểm chẩn đoán và tiên lượng kém. Phẫu thuật đơn thuần được xem là điều trị tiêu chuẩn cho giai đoạn sớm; điều trị đa mô thức kết hợp phẫu thuật, xạ trị và/hoặc hóa trị được khuyến cáo cho giai đoạn tiến xa. Trên thế giới và tại Việt Nam còn ít hướng dẫn điều trị và dữ kiện về bệnh lý này.
Phương pháp: Chúng tôi báo cáo một trường hợp bệnh nhân ung thư niệu đạo nguyên phát và tổng quan y văn về chẩn đoán và điều trị ung thư niệu đạo, tập trung đến hóa xạ trị đồng thời triệt để cho giai đoạn tiến triển tại chỗ tại vùng.
Kết quả: Bệnh nhân nữ 48 tuổi, được chẩn đoán ung thư niệu đạo cT4N2M0, giải phẫu bệnh loại carcinôm tế bào gai. Bệnh nhân được hóa xạ trị đồng thời triệt để tổng liều 64 Gy/ 32 phân liều kết hợp hóa trị 5-FU 800 mg/m2 và Cisplatin 80 mg/m2 tổng 4 chu kỳ, đạt được đáp ứng lâm sàng khả quan với độc tính chấp nhận được.
Kết luận: Hóa xạ trị đồng thời triệt để là mô thức điều trị hiệu quả cho ung thư niệu đạo nguyên phát loại carcinôm tế bào gai giai đoạn tiến triển tại chỗ tại vùng.
Abstract
Introduction: Primary urethral cancer is a rare malignancy, accounting for less than 1% of genitourinary cancers. It is often diagnosed at an advanced stage, with lymph node metastasis, and has a poor prognosis. Surgery is the standard of care for early-stage disease, while multimodality treatments combining surgery, radiotherapy, and/or chemotherapy are recommended for locally advanced cases. However, clinical evidence and treatment guidelines remain limited worldwide, including in Vietnam.
Methods: We report a case of locally advanced primary urethral squamous cell carcinoma treated with definitive concurrent chemoradiation therapy, accompanied by a literature review on diagnostic approaches and focused on concurrent chemoradiation therapy.
Results: A 48-year-old woman was diagnosed with primary urethral squamous cell carcinoma, staged cT4N2M0. She received definitive concurrent chemoradiation therapy with the dose of 64 Gy in 32 fractions combined with chemotherapy (5-fluorouracil 800 mg/m² and cisplatin 80 mg/m² for 4 cycles). The patient achieved a favorable clinical response with manageable toxicity.
Conclusion: This case highlights the potential role of definitive concurrent chemoradiation therapy as an effective and tolerable treatment option for locally advanced primary urethral squamous cell carcinoma.
