ASSOCIATION BETWEEN KRAS, NRAS, AND BRAF MUTATION STATUS AND CLINICOPATHOLOGICAL CHARACTERISTICS IN METASTATIC COLORECTAL CANCER AT HO CHI MINH CITY ONCOLOGY HOSPITAL

Mục tiêu: (1) Xác định tỉ lệ đột biến gen KRAS, NRAS, BRAF ở bệnh nhân ung thư đại trực tràng di căn; (2) Xác định mối liên hệ giữa tình trạng đột biến gen với một số đặc điểm bệnh học của bệnh nhân ung thư đại trực tràng di căn.
Đối tượng và phương pháp nghiên cứu: Hồi cứu mô tả loạt ca 356 trường hợp ung thư đại trực tràng di căn được chẩn đoán và điều trị tại Khoa Nội tuyến vú, tiêu hóa, gan, niệu tại Bệnh viện Ung Bướu TP.HCM. Giải trình tự phát hiện đột biến gen KRAS, NRAS và BRAF và mối liên quan với một số đặc điểm bệnh học. Kết quả: Trong 356 trường hợp khảo sát gen KRAS, NRAS và BRAF chúng tôi ghi nhận tỉ lệ đột biến gen lần lượt là 50%, 3,8%, 5,6%. Đột biến gen KRAS thường gặp ở bệnh nhân có vị trí u ở bên trái và trực tràng với p = 0.038 và 0.004 . Đột biến gen BRAF thường gặp ở bệnh nhân trên 60 tuổi với p = 0.013, những bệnh nhân có nồng độ CEA huyết thanh tại thời điểm chẩn đoán > 5ng/ml với p = 0.04. Các đặc điểm bệnh học khác như giới, thể trạng, độ mô học và số lượng tạng di căn liên quan không có ý nghĩa. Không tìm thấy mối liên quan có ý nghĩa giữa tình trạng đột biến gen NRAS và các đặc điểm bệnh học.
Kết luận: tỉ lệ đột biến gen KRAS ở bệnh nhân Ung thư đại trực tràng di căn tại bệnh viện Ung Bướu TP.HCM là 50%. Đột biến gen KRAS liên quan có ý nghĩa với vị trí u, tỉ lệ xuất hiện cao ở vị trí u bên trái và trực tràng. Tỉ lệ đột biến gen NRAS là 3,8%. Tỉ lệ đột biến gen BRAF là 5,6%. Đột biến gen BRAF liên quan có ý nghĩa với nồng độ CEA huyết thanh tại thời điểm chẩn đoán (>20ng/ml) và độ tuổi (>60 tuổi).
Abstract
Objectives: (1) To determine the mutation rates of KRAS, NRAS, and BRAF genes in patients with metastatic colorectal cancer; (2) To investigate the relationship between gene mutation status and certain clinical characteristics of patients with metastatic colorectal cancer.
Subjects and Methods: This study is a retrospective descriptive analysis involving 356 cases of metastatic colorectal cancer diagnosed and treated at the Department of Breast, Gastrointestinal, Liver, and Urology at Ho Chi Minh City Oncology Hospital. Gene sequencing was performed to detect mutations in KRAS, NRAS, and BRAF, and to assess their associations with various clinical characteristics.
Results: Among the 356 cases examined for KRAS, NRAS, and BRAF mutations, the observed mutation rates were 50% for KRAS, 3.8% for NRAS, and 5.6% for BRAF. KRAS mutations were most prevalent in patients with tumors located in the left colon and rectum, with p-values of 0.038 and 0.004, respectively. BRAF mutations were more frequently observed in patients over the age of 60 (p = 0.013) and those with serum CEA levels greater than 5 ng/ml at the time of diagnosis (p = 0.04). Other clinical characteristics such as gender, performance status, histological grade, and the number of metastatic sites did not show significant correlations. No significant association was found between NRAS mutation status and clinical characteristics.
Conclusion: The mutation rate of the KRAS gene in patients with metastatic colorectal cancer at Ho Chi Minh City Oncology Hospital is 50%. KRAS mutations are significantly associated with tumor location, being more frequently found in tumors located on the left side and rectum. The mutation rates for NRAS and BRAF are 3.8% and 5.6%, respectively. BRAF mutations are significantly correlated with serum CEA levels at diagnosis (>20 ng/ml) and age (>60 years).
